Before your initial evaluation:

• Please call our office to verify your insurance benefits. 
• Fill out our patient forms to the best of your ability before coming in.
• Arrive at least 15 minutes early for your first appointment.
• Bring with you a copy of your insurance card and photo ID. 
• Wear comfortable clothing that is easy to move around in allows the physical therapist to access your point of pain.

What to expect on your first visit:

At Momentum Physical Therapy, you will receive the attentive physical therapy you deserve!

Our expert physical therapist will perform a comprehensive evaluation which usually lasts approximately 60-90 minutes. During the evaluation, your physical therapist will ask questions about your medical history, current pain or injury. Your evaluation will include an assessment of your movement, strength, coordination, flexibility, balance and more.

After your evaluation, your physical therapist will thoroughly explain your personalized treatment plan and expectations for recovery. There will be some treatments performed the first day to get you started on your road to recovery. However, during the next visit you will get into the full swing of your treatment plan.

The amount of therapy needed will be discussed with you, depending on the recommendations of your physical therapist. We offer many convenient appointment times, however prime times can become busy, so it is best to book ahead as much as possible.
