• Examine range of motion
• Assess muscles and joints
• Education regarding your injury
• Discuss the best treatment plan

Terms and Conditions:

Our free injury screen is for a complimentary physical therapy screening only. The goal of the screening is to determine whether a person would benefit from a physical therapy evaluation or referral to another health care professional. Screenings are provided by a licensed provider, however are not inclusive of a comprehensive physical therapy evaluation or treatment.

Our free injury screen program has no cash value and is not for use in conjunction with any insurance program, nor is it a substitute for any portion of patient financial responsibility (co-pay, co-insurance, or deductible) under any insurance plan of benefits.
